Notice of Provider Change & Service Cancellation

I authorise Lowie Harrison and Completeworxz Limited (Complete Trash) to act on my behalf in relation to my current waste collection service.

This authority includes - Contacting my current provider, requesting relevant account and contract information, giving formal notice of cancellation, confirming termination dates and final charges, receiving written confirmation of cancellation.

This written authority is provided for the purposes of the Privacy Act 2020 and permits my current provider to release account information necessary to process this cancellation.

CANCELLATION INSTRUCTIONS

Please cancel my existing waste service with my current provider at the earliest possible date.

Please have my current provider delete all my personal information.

I understand I remain responsible for any outstanding or contractual charges payable to my current provider.
